MEMBERSHIP AGREEMENT
Sovereign Care Collective (SCC)
Effective Date: January 7, 2026
This Membership Agreement (“Agreement”) is entered into between you (“Member”) and Sovereign Care Collective (“SCC,” “we,” “us,” or “our”).
By joining SCC, you agree to the terms outlined below.
1. Private Membership
By joining Sovereign Care Collective (“SCC”), you agree to become a member of a private membership association. This agreement governs the relationship between you (“Member”) and SCC.
SCC is a voluntary, member-based community designed to support a proactive, wellness-focused approach through shared contributions.
2. Not Insurance
SCC is not insurance, a health plan, or a healthcare provider.

There are no claims, approvals, or guarantees of payment

Membership does not replace insurance

Membership does not satisfy any federal or state insurance requirements Members are solely responsible for obtaining any insurance coverage they deem necessary.
3. No Medical Relationship
SCC does not provide medical advice, diagnosis, or treatment.
Nothing within SCC, including resources, directory listings, or communications, establishes a doctor-patient relationship.
Members are responsible for all health decisions.
4. Independent Practitioners & No Liability
All practitioners accessed through SCC are independent.

SCC does not employ, recommend, or endorse practitioners

SCC does not oversee or control services provided
Members are solely responsible for selecting and working with practitioners.
SCC:

Does not mediate disputes

Does not investigate practitioner conduct
SCC shall not be liable for any claims, damages, or losses arising from member interactions with practitioners.
5. Member Contributions
Members agree to make recurring monthly contributions.

Contributions support a shared community pool

Contributions are not payments for services

Contributions do not guarantee reimbursement
A portion of contributions may be used for administrative purposes.
6. Reimbursement Consideration
Members may submit eligible expenses for reimbursement consideration.
Reimbursements:

Are not guaranteed

Are reviewed based on:
○ Eligibility
○ Available funds
○ Community sustainability
All reimbursements are made at the sole discretion of SCC.
7. Reimbursement Process

Members pay for services directly

Receipts must match the member’s name

Requests are submitted through the member portal
Requests are reviewed quarterly and receipts submitted by:

March 31

June 30

September 30
-
December 31
A 5-day grace period applies for end-of-quarter services.

After the 5-day grace period, receipts are no longer eligible for processing
8. Limits
Each membership has a quarterly limit based on contributions, with up to an additional 15% support from the community pool.
Current limits:

Individual: up to $300
-
Couples: up to $525 (shared)

Family: up to $800 (shared)
Limits are not guaranteed payouts.
9. Waiting Period
New members must complete a 90-day waiting period before submitting requests.
10. Guidelines
All reimbursement considerations are made in accordance with SCC Member Guidelines.
Guidelines may be updated at any time to maintain sustainability and clarity.
11. Review of Requests
Members may submit additional information for a one-time internal review.

Final decisions remain at SCC’s sole discretion

SCC is not obligated to provide appeals or dispute resolution processes
12. Privacy
Member information is kept private and used only for administrative purposes.
SCC does not sell or share personal data.
13. Cancellation
Members may cancel at any time.

Cancellation takes effect at the end of the billing cycle

Membership must remain active through the applicable quarterly review date for any submitted requests to be eligible for consideration
